Skilled Nursing services at Vine Homecare & Staffing, Inc. begin with an individualized care plan developed by our health care team. During the development of the care plan, we coordinate with the patient, family members and the physician.

We then assign a Registered and Licensed Practical Nurse to deliver primary care to patients who have recently been discharged from the hospital or have been home-bound for most of their lifetime. We know that the transition from one lifestyle to another one that involves nursing care can be intimidating but we make the process as smooth as possible.

We care for patients who need home care or acute nursing care. We also take care of patients who are chronically ill.
